Wisdom Tooth Extraction

Wisdom teeth do not always need to come out. When they are impacted, crowded, or causing problems, removal may be recommended after a careful exam and X-rays.

Oral surgery procedures at West End Dental Centre, including wisdom tooth extractions, are performed by our general dentist team. Our dentists are not licensed specialists in oral and maxillofacial surgery.

What are wisdom teeth?

Wisdom teeth are the third and final set of molars at the very back of the mouth. They usually begin to come in between the late teens and mid-twenties, long after the rest of the adult teeth are in place.

Many jaws do not have enough room for them. A wisdom tooth that cannot fully emerge is called impacted. It may be tilted toward the neighbouring molar, lying on its side, trapped beneath bone, or only partly through the gum. Some people never develop wisdom teeth at all, while others have them come in straight and healthy.

When removal may be recommended

Not every wisdom tooth needs to be removed. Some can simply be monitored with periodic X-rays. Removal may be recommended when a wisdom tooth is:

  • Causing repeated infection or swelling of the gum around a partly erupted tooth
  • Decayed, or contributing to decay or damage on the neighbouring molar
  • Associated with gum disease or bone loss around the back teeth
  • Linked to a cyst or other changes seen on X-rays
  • Causing ongoing pain, pressure, or difficulty keeping the area clean

Symptoms like these can have other causes. Only an exam and X-rays can confirm whether your wisdom teeth need attention.

Why age matters

Wisdom teeth are often assessed in the late teens or early twenties. At that stage the roots are usually not fully formed and the surrounding bone tends to be less dense, which can make removal more straightforward and healing quicker for many people.

This does not mean everyone should have their wisdom teeth removed early. It simply means a timely assessment gives you and your dentist more information to make a decision, rather than waiting until a problem develops.

What to expect on the day

Every case is planned individually, but most wisdom tooth extractions follow these steps:

  1. Reviewing the plan

Your dentist reviews your X-rays and explains how the tooth will be removed, what to expect, and how to prepare.

  2. Local anesthetic

The area is thoroughly numbed. You may feel pressure, but if you feel pain at any point, tell us so we can adjust.

  3. Removing the tooth

An erupted wisdom tooth may be removed much like any other extraction. An impacted tooth may require a small incision in the gum, and the tooth may be divided into sections for easier removal.

  4. Stitches and gauze

Stitches may be placed if needed, and gauze helps a blood clot form in the socket.

  5. Aftercare instructions

You receive written instructions for the coming days before you leave.

If you feel anxious about the procedure, let us know. For extensive treatment or significant anxiety, general anesthesia may be discussed, although not every patient or procedure is a candidate.

Recovery

Some swelling and discomfort are normal and often peak around the second or third day. Cold compresses on the cheek, soft foods, rest, and pain relief as directed by your dentist can help. For the first few days, avoid smoking, drinking through a straw, and forceful spitting, as these can disturb the blood clot and lead to dry socket. Gentle warm salt water rinses are usually started after the first 24 hours.

Many people return to their normal routine within a few days, while the socket itself continues to heal over the following weeks. Contact us if you have heavy bleeding that does not ease with pressure, a fever, or pain that worsens after the third day.

When we may refer you to an oral surgeon

Our general dentists remove many wisdom teeth, but some cases are better managed by an oral and maxillofacial surgeon. Referral may be recommended when a tooth is deeply impacted, sits close to the nerve in the lower jaw, has complex root shapes, or when your medical history calls for a different setting.

If referral is in your interest, we will explain why and help you understand the next steps.
